Student-Athlete Information Forms

While at Simpson, the athletics communication office will maintain an online biography of each athlete including statistics, honors, awards and achievements. In order to do so, we ask that each athlete fill out a questionnaire before they arrive on campus. After your freshman year, you need only complete a short form in order to keep your information accurate.
